Some of the bones are marked with a red asterisk (*) - in these instances, the location on the bone should be specified, if possible, as follows:

If the tumor was located in the femur, fibula, humerus, radius, tibia, or ulna was it:  

  • proximal  (at the end of the bone that is close to the body)

  • distal (at the end of the bone that is furthest from the body)

  • or diaphysis (in the main shaft of the bone).

If the tumor was located in the scapula, was it:  

  • proximal (at the top of the scapula, near the peak of the shoulder blade)

  • or distal (at the bottom of the scapula, near the head of the humerus).
